ESWAVAX - Submitting a closing report

Before you start - Before you can submit an ESWAVAX closing report, you will need the following:

1. A device that has a browser to connect to the internet (Google Chrome, Safari)

2. Connection to internet via Wi-Fi or mobile data

3. Active ESWAVAX credentials for signing in

4. Be in an active vaccination site (Had submitted an opening and arrival report)

Step 1 - Submit a departure report

Once you are done vaccinating on site, you must submit a departure report. To submit a departure report, you need to press "Daily report" button within the daily report screen then the "Depart" button below your active site (if you have multiple sites). You can cancel departure if you want to proceed with vaccination on the same site without having to arrive again.

Step 2 - Transfer inventory

Once done vaccinating for the day, you need to transfer all used items to the used bin of your team.

  • Login to supply chain - Open your browser and enter address app.luvelo.org/supplychain. On the sign in page, choose "Login with Microsoft". Input your credentials (username & password) then sign in. If asked if you want to "Stay signed in?' choose no.

  • Create an item transfer - Once you have successfully logged in, you can now create an item transfer. Select "Item Transfers", then the "+" sign on the top right corner.

From there you can fill in the required information:

Origin bin - this should be the bin/team you are transferring from

Destination bin - this should be the bin/team you are transferring to

Items to transfer - this is the list of all the items you are transferring

Photos of items to transfer - take photos of items you are transferring especially the vaccine vials

Memo - the reason for the transfer

  • Choosing items to transfer - Once you have selected your origin and destination bin. Select "Items to transfer". You can check the "select all items" box if you want to transfer all listed items or check the box next to the item(s) you want to transfer. Once checked the box, then select "Add items" at the bottom.

  • Adjusting quantity to transfer - To adjust the quantity of the item(s) you want to transfer, press the pencil icon next to the item. You can enter the number of the quantity you want to transfer on the "amount to transfer" bar. Once you have entered the quantity then press "save". You can repeat this until you have adjusted all the quantities of the items you want to transfer then press the back sign on the top left corner to return to the transfer screen.

  • You can then attach photo of items and signature, then submit.

Step 3 - Submit closing report

Once you have transferred your inventory, you need to switch from the supply chain app back to ESWAVAX. You can then submit a closing report. Go to "daily reports" then select "close report". You will then be required to fill in closing report information. Once you have filled out all the information, you can then press "submit"

Re-opening daily report - If you want to proceed with vaccination yet you had submitted a closing report, you can re open the report without having to submit a new opening report. Press the 3 bars on the top left corner then select "reopen daily report'' and confirm. You can the cancel departure to vaccinate on the same site.
