Purpose: To create dynamic forms that are used to capture information within the organization.
Scenario: You are conducting a survey to assess user satisfaction with the newly deployed application. To create the survey form, use the Luvelo Forms application.

This article will show you how to create a form in the Forms Admin application.
Form Access
Click on New Form to begin the form creation process.
Form Fields
Click Fields tab to access the form actions screen.
Click the "Pencil Icon" to edit form information:
Add form details:
Click "+ Section" to add a new form section
Section - Logical grouping of related fields within a larger form.
Section Title - Descriptive heading or label that identifies a specific section of a form
Section Order - Sequence in which the different sections of a form are presented to the user.
Click "+ Field" to add a form section filed.
Field - Individual element within a form where a user can input data.
Label - Descriptive heading or label that identifies a specific field of a form.
Type - Specific kind of data an input element is designed to collect.
Mandatory - Field must be competed before the form can be successfully submitted by user.
Is Active - Field will show on form.
Is URL Parameter - Field content will be populated by the system and not the user and thus sent to the backend as a URL parameter.
Task Key - Users response to the field will be included in the Task Title.
Link Icon (Dependency) - The filed relies on or is controlled by another fields input.
Bell Icon - Will send notification based on the filed input against set condition (conditional notification).
Setting Conditional Notification
When Value - What value should trigger the conditional notification.
Notification type - Set if the notification should send to Slack or SMS. If Slack > Choose Slack channel, if SMS > enter phone number.
Send full response - Toggle on if you want the notification send to be the form response (this includes the question and the response provided).
Notification Message - Specify what message should be included in the notification when it sends.
To arrange the order of the fields, first click the pencil icon on the field you want to move. Then, click and drag the three horizontal lines icon to reposition it.
Header - Description that identifies what the form is for.
Form Footer - Located at the very bottom of a form.
Delete - Removes field or section from the form.
6. Click the "Save Button" to save form information.
7. Click on the "Eye" Icon to preview form.

Actions
Click Actions tab to access the form actions screen
Click "Send to Slack" to configure form submission to Slack.
Send to Slack - You can set the slack channel you want the form data to send to on submission
Generate PFD on Submission - You can toggle this on if you want the form submission data to be in a pdf format as well.
Click "Send to SMS' to configure form submission confirmation to SMS.
SMS Confirmation Message - Set the message you want the user to receive as an SMS confirmation after submitting the form.
Send QR Code in SMS - Toggle on if you would like the user to receive a link in the SMS to a screen that shows a QR Code.
Click "Create Task" to configure task creation when form is submitted.
Task List - Choose what Luvelo task list you want form submission data to be created under.
Form Submitter Config - State how the form submitter will be associated with the task that is created on submission
Tasks Status - Set the status you want the form submission data task to be on when form is submitted.
Task Title - Set the task title you want to be given to the form task when being created on submission.
Slack Channel - Set the Slack channel you want the notification for the task created on submission to be sent to.
Fallback Assignee - This is the person to whom the task created on submission will be assigned if the designated initial assignee is unavailable or inactive.
Tasks Assignee - This is the person to whom the task created on submission will be assigned.
At Work Group - The task created on submission will be assigned to a member of the selected 'At Work Group'.
Click "Send Wifi Voucher" to configure sending of Wifi voucher to form submitter when form is being submitted.
SMS Wifi Message - Set the Wifi voucher SMS message content and include Wifi [Username] and [Password] within the message.
Configurations
Click Configurations tab to access the form actions screen.
Click "Configure Countdown" to set form countdown during filling out.
Time to Expire (minutes) - Set the number of minutes a user has to complete the form.
Show Hours - Show the form timer hours.
Show Minutes - Show the form timer minutes.
Show Second - Show the form timer seconds.
Click "Confirmation Message" to configure form submission confirmation message user will see when submitting the form.
Submission Confirmation Message - This is the message the user will see when submitting the form. If empty then the default will be "Submission Successful".
Administrators
Click Administrators tab to access the form administrators screen.
Administrators - Users who have access to editing the form.
Can View Results - Users who have access to editing and viewing form results.
Can not View Results - Users who have access to editing but not have access to viewing results.
Click "Add Administrators from Group" to add users who are part of a group.
Choose group with users you want to add as administrators.
Toggle on "Is all users allowed to view results" if you want users to view form submission results and click "add all group users as administrators" to save.
Click "New Administrator" to add a user as the form administrator.
Choose the user you want to add as administrators.
Toggle on "Is user allowed to view results" if you want user to view form submission results and click save.
Click 'Remove view results access' to revoke a user's permission to see form submission results.
Click the "Bin Icon" to remove user from form administrators.
Additional Action
To do more action, click the 3 dots.
Import Configs - This feature allows you to import existing form configurations from a JSON file.
Click "Import Configs" to start the process.
Choose the file you want to import form your storage and click "upload'.
You will then get "form successfully created" message.
Export Configs - This feature allows you to export form configuration into a JSON file.
Click "Export Configs" to start the process.
You will then get a warning pop up, read carefully and click "Export" to proceed. Your form JSON file will be downloaded to your device.
You will then get "configs successfully exported" message.
Share - This feature allows you to copy the form URL. You can then share this URL with users so they can access, complete, and submit the form.
Click "Share" to copy form URL to your device clipboard.
You will then get the success message.
