Self Check-in

Purpose: With the Self Check-in application, you can complete the check-in process before meeting the healthcare worker or arriving at the healthcare facility. This significantly reduces wait times. Additionally, you can enter your own information, at the comfort of your space. Ultimately this promotes the confidentiality of your data.

Application Roles Required:

  • None

Key Words:

  • OTP (One Time Pin) - This is a short code that you will receive on your device via sms and can be only used once.

  • Next of kin - Refers to a person’s closest living relative or relatives.

You are encouraged to fill in all the required information on the screens as they will help the health workers to offer quality services to you. Please note that during the registration process you are only able to proceed to the next screen if all the required fields are entered. If there is missing information on the screen, the 'Continue' or 'Next' button will be greyed out (deactivated).

Follow the steps in order to do self check-in:

  1. With your mobile device, open the browser and use the link app.luvelo.org/myhealth.

  2. Click the 'Log in' button to start the the check-in.

  3. Enter your phone number and click 'Continue'. Make sure to use the phone number that you have with you.

  4. Click the 'Verify phone number' button. This will send the an OTP on your mobile device.

  5. Enter the OTP/code that you received on your phone via sms.

  6. A list of patient files which are possible matches will show. Check carefully and select your correct file.

  7. Click the 'Check-in' button.

  8. If you have a token, click 'Scan Token' and scan the QR Code on the token, and if you do not have the token, click 'Continue without token'.

  9. Click the site you are getting the service from and click 'Continue'.

  10. Take a selfie photo. Make sure that your face is well captured.

  11. If you have a scheduled appointment, click on the appointment then you click 'Continue'. If you have no appointment, click 'Check-in without appointment'.

PATIENT OVERVIEW

On the patient overview page, you will find different screens. Screens marked with a red “X” indicate that some information is still missing and needs to be entered. Screens marked with a green checkmark indicate that all required information has been entered. Please review and enter accurate information on all screens to ensure you receive quality healthcare.

The Patient Overview has the following:

General information screens which includes:

  • Contact info - On this screen you enter your phone numbers. Once you enter the primary phone number , click 'Send SMS Code' button. You will get an OTP that you need to enter to verify the phone number. The primary phone number and at least one next of kin is required.

  • Permanent address - Enter all the required address details.

  • Temporal address - This is your secondary place of residency apart from your home. Reasons for staying on the temporary address can be due to work, school or other commitments. If you do not have a temporary address, you can select the "same as permanent address" box. If the box is selected, the information from the permanent address screen will auto populate.

  • Background info - Enter your general background information. Fields that have a star on them are mandatory (they can not be skipped).

  • Occupation info - Enter all your occupation information on this screen. This includes your education level, employment status, occupation and your workplace details. If your workplace name is not showing on the dropdown list, you can choose other to specify.

    Note: If you are a health worker or a staff member for TLC, on 'Occupation' choose 'Health Worker'. This will hide your name and critical medical information from non essential medical staff.

  • Photo of ID - You can select the type of identity document you have and attach a photo. It can be either a national ID or a Passport. If none of those, then you can chose none and the reason.

Medical history information which includes:

  • Surgical history - Add all the surgeries you once had before. You have to include the surgery date when listing the surgery.

  • Social - This record your social life including history of smoking, drinking, drugs and use of condoms.

  • Medical History - This captures all general medical conditions that you might have faced.

Screenings which include:

  • TB Screening - This is screening for TB. If the answer is YES on any of the questions, specify the number of days you have been experiencing that symptom.

  • STI Screening - This is screening for STIs. If the answer is YES on any of the questions, specify the number of days you have been experiencing that symptom.

  • Obstetric - These screening questions are only available for female patients. They relate to childbirth and details associated with it.

  • HIV Screening - Enter your HIV status if you were once tested before. Please enter an accurate date for last HIV test. If you do not know your status, click 'Unknown'.

Today's appointment which includes:

  • Complaints - This is where you list the symptoms or any medical complaints you currently have. You can pick the symptom(s) from the available list or choose other to specify. If there are no symptoms, then you can choose the no symptoms option.

  • Medication History - This is where you list the current medication you are taking.

Once you are done entering all information and all screens have green check marks, click 'Finish'.

You are now done with self check-in. If you want to update your information that you have entered, click 'Update existing check-in' button.

Note: If you are a healthcare worker, it is going to require your permission for another general healthcare worker to access your file in Outpatient.
