Follow-Up Cases

Purpose: Regular follow-ups and documentation allow healthcare providers to monitor patients’ progress, make necessary adjustments to treatment plans, and address any complications early, thereby improving patient outcomes.

Application Role Required:

  • CaseManager

Follow the steps to follow-up on cases:

  1. Log in Outpatient application.

  2. On the home screen, click 'Follow-up Cases'.

  3. You can filter cases you want by case manager, case type or case status.

  4. Click on the patient file you want to follow-up on.

  5. Select the case and click the pencil icon to start editing.

  6. Click the Regimen tab to see all regimens associated with the patient. To add more, click '+ Regimen'.

  7. Click the Appointment tab to see all appointments associated with the patient. To add more, click '+ Appointment'.

  8. Click Facility History tab to document the regimen transfer information.

    Date - Enter date when regiment was transferred.
    Original Facility - Enter facility where patient is transferring from.
    Destination Facility - Enter facility where patient is transferring to.
    Notes - Enter reason why patient is transferring.
