Purpose: To register VIPs in order for them to get medical services.

You are encouraged to fill in all the required information on the screens as they will help the health workers to offer quality services to the. Please note that during the registration process you are only able to proceed to the next screen if all the required fields are entered. If there is missing information on the screen, the 'Continue' or 'Next' button will be greyed out (deactivated).

Application Roles Required:

  • Clerk

  • Patient

Follow the steps to start the check-in process:

  1. Log in Med Check-in app.

  2. Select the site. Make sure you are selecting the correct site.

  3. If patient has token click the Scan token button in order to link it with the patient visit. If there is not token click Continue without token.

  4. Search for patient file using the provide parameters, EVAX ID, PIN, Phone, Birthday and Name. Make sure the phone number provided is the one that they are currently using.

    If the patient has visited TLC before you can click Yes and if it is his first time click No. After that click Continue to go on the next screen.

  5. Take a photo of the patient.

    • If a care giver is visiting to get medication or any other type of service on behalf of the patient, click Caregiver then take a photo.

    • If the patient died on way to the health facility, click DOA. This should not allow to take any photo.

  6. From the list of patient shown, identify the patient and select. Make sure you confirm with the patient if all the details like last four digits of the phone number, EVAX ID, National ID and last visit date are correct. If the patient is not listed, click None of these patients, expand search to perform another search. After identifying the patient click Continue.

  7. Enter the patient details on the search for patient screen. You are advised to enter all the information correctly. Enter all the information even if it's not required. This ensures quality health care services to be provided to the patient. Click Continue when you are done.

  8. If the patient file is new, click Add New Patient button.

Patient Overview

In the patient overview page, we have different tabs with different patient information. All the patient information in each tab need to be accurately filled out. We have mandatory and optional information.

Mandatory - The fields with mandatory information are marked with stars*. They are a must to be accurately filled out and can not be skipped.

Optional - Can be filled out if information is available or can ben skipped.

The patient overview page has

  1. General patient information which includes

  • Contact info - Primary phone number is required and at least one next of kin. To verify the patient phone number click Send SMS code. This will send an OTP via an SMS to the the patient phone number. Enter the verification code on the provided field. If the patient does not have the phone with him or the battery is dead, click Request Exception. This will send a request message on slack which the senior med team will approve or reject.

    Click the Add button to add the next of kin. Enter all the information as required.

  • Permanent address - Enter the home address of the patient. The region auto populates based on the selected Inkhundla.

  • Temporal address -Temporal address - This is the secondary place of residency apart from the patient's home. Reasons for staying on the temporary address can be due to work, school or other commitments. If the patient does not have a temporary address, you can select the "same as permanent address" box. If the box is selected, the information from the permanent address screen will auto populate.

  • Background info - This is the general background information for the patient. Fields that have a start on them are mandatory (they can not be skipped).

  • Occupation info - This is information about the patients occupational and educational status. If they are employed or students, more information like workplace name and school name will be required. If those are not in the dropdown list, you can choose other to specify.

    • Note: If the patient is a health worker or a staff member for TLC, on 'Occupation' choose 'Health Worker'. This will flag the patient's file as confidential. Confidential visits are only found in the confidential site. Only Senior medical team and a select of users are allowed to have access to confidential files.

  • Photo of ID - You can select the type of identity document the patient has and attach a photo. It can be either a national ID or a Passport. If none of those, then you can chose none and the reason.

2. Medical history which includes

  • Medical History- This includes all the medical history including chronic diseases. If the patient once had an STI or TB before, an HIV screening will be required during registration.

  • Surgical history - Add surgeries the patient has had in the past. You have to include the surgery date when listing the surgery.

  • Social - This the patient's social life including history of smoking, drinking, drugs and use of condoms.

3. Screening which includes

  • Tuberculosis - This is screening for TB. If the answer is YES on any of the questions, specify the number of days you have been experiencing that symptom.

  • STI - This is screening for STIs. If the answer is YES on any of the questions, specify the number of days the patient has been experiencing that symptom.

  • HIV - This screen is triggered if the patient screens positive for TB and STI screening
    Enter the patient HIV status if patient once tested before. Please enter an accurate date for last HIV test. If patient do not know his status, click 'Unknown'.

  • Adult Risk Assessment: This screen shows if a patient has a positive STI screening.

  • Obstetric - These screening questions are only available for female patients. They relate to childbirth and the process associated with it.

4. Today's appointment which includes

  • Complaints - This is where you list any health complaints the patient has You can pick the symptom(s) from the available list or choose other to specify. If there are no symptoms, then you can choose the no symptoms option.

  • Medication History - This is where you list current medication the patient is taking.

  • Vitals: Enter patient vitals.

  • Visit Overview - This screen shows an overview of the today's visit. Under the Check-in Summary section it will show you all complaints the patient has, Medical History, TB Screening results and HIV screening recommendations.
    Upcoming Appointments will b shown under Appointments section.

    You can also specify the services that they came for on the list of services. Some services will be added automatically based on the screening results.

Once you are done with registration, you can click 'Submit Check-in'.
