Purpose:
Quizzes also provide a means of assessment, helping to identify areas where further training might be needed. Writing quizzes also enhances training by reinforcing key concepts and procedures, ensuring what has been taught, to be retained effectively. Immediate feedback from quizzes can highlight the effectiveness of training sessions.
Follow the steps to create quizes in Form Builder application.
Log in to the Form Builder application.
Click "New form" button to start creating form.
Form configuration details:
Form Name: Enter the name of the quiz you are creating.
Form Type: Select “Quiz” to differentiate it from a general form.
Custom Actions: Define advanced custom actions for the form:
Enable Countdown: Activate this action and configure the countdown time for quiz completion.
Send Submission To Slack: Edit the Slack channel where quiz submissions will be sent.
Generate PDF: This will generate a PDF of the quiz results and send it to the configured Slack channel.
Score type: Configure how you want to show the score.
Activate the form: Make the form visible to users by activating it.
Countdown Configuration: This is the countdown that will show on the quiz form.
Add sections and fields:
Click “+ Section” to add a section and enter the section name.
Click “+ Field” to add fields for quiz questions.
Configure the questions:
Label: Enter the quiz question.
Type: Select the type of answer (e.g., Text, Long, Number, Date, Dropdown, Multiple Tags, Radio Button, Phone Number, Asset Dropdown).
Options: Click the pencil icon to add multiple answer options for answer types like Dropdown, Multiple Tags and Radio button. Multiple tags allows you to add multiple correct answers.
Mandatory: Tick the “Mandatory” checkbox if the question is required.
Is Active: Tick the checkbox to activate the question.
Field Answer: Define the correct answer and its value for quiz evaluation.
Save the quiz form.
