Purpose:

The DREAMS (Determined, Resilient, Empowered, AIDS-free, Mentored, and Safe) service aims to reduce HIV/AIDS among adolescent girls and young women.

This article will guide you on how to effectively implement and record the DREAMS services within Luvelo Outpatient application. It will cover the necessary steps to ensure accurate documentation, streamline service delivery, and enhance patient outcomes.

Follow the step for documenting Dreams service in Outpatient:

  1. Log in Outpatient.

  2. Search for the patient file. You can search for the patient file using any of the following methods:

    • Visits - This shows you a list of all visits for the day. If patients are not checked out in the system, the visit automatically gets checked out after 24 hours.

    • All Visits - Here you can search for patients using Phone number or Evax ID only. It shows you patient visit history.

    • Recent Activity - This shows you a list of files that you have recently opened and the time you accessed the file.

  3. Go to the visit screen and add the Dreams service.

OVERVIEW TAB

  1. Patient Details Section

    • Dreams Client ID - Enter patient Dreams ID

  2. Visit Details Section

    • Referral Number - You get the referral number from the dreams form.

    • Referral Partner - Enter partner who referred the patient.

    • Referral Date - Enter date when patient was referred.

  3. HIV Section

    • Status - Enter HIV status for patient if know.

  4. Consent Section

    • Consent to share records - If patient consent, click yes, and sign else click no. By signing the form the patient acknowledges that they have read all the terms and conditions.

  5. PrEP Section

    • Reason not on PrEP - If patient is not on PrEP enter reason why they refused.

  6. Condoms Provided

    • Search for condoms to be dispensed.

SERVICES TAB

  1. Service Referred Section

    • Add services that were referred by the partner

  2. Services Provided

    • Add services that were provided to the patient.

  3. Notes Section

    • Add if there are any additional notes.

  4. Advocacy Outcome

    • Select PrEP, VIA and Family Plan advocacy outcome.

REFERRAL FORM TAB

  1. Referral Form Photo

    • Attach he referral form photo.

SERVICE PROVIDER

Click "Select Service provider" link, followed by clicking by clicking "Set As Me" to show your name as the service provider.

SUBMIT REVIEW

Finally click the "Submit Review" button to send the service template for review.
